Transitions Program

The Transitions Program serves individuals between the ages of 14 and 24 who have mental health, drug and/or alcohol issues and who are at risk of being or who are homeless. This Program offers case management, support and advocacy services. There is a full time case manager located in the Clifford Beers Center.
